Emailing patch authors/mailing list on state change

May need to be configurable per mailing-list/author: it's not really a
feature I'd need and 'accepted' is not a state changes people would
particularly care about in my workflow. 'Awaiting upstream' would be
more useful but you'd need the email to contain per-project configurable
text to accompany the state change and explain what it means.  You'd
probably need a table per-project of which state changes trigger notifications.
