[PATH] rpaphp doesn't initialize slot's name

Hi Greg,

This patch includes some changes that I missed when I cut
php_phy_location.patch.
It fixes the kernel crash when rpaphp is loaded because slot->name is
not initialized.

Please apply it to your tree.
