merged to master as of a024fb7daa0db6d54d916b8885b28cdfe9a119c6.

however, I've then very heavily reworked it.

there is still no mention of:
1) how the notification mechanism works (or reference to where it's
documented)
2) What are the restrictions on calling each of the OPAL calls.
   These should *not* reference internal OPAL state machines that are
   FSP specific.
