patch list action bar design changes

Thanks for the feedback! I've implemented the mockup (attached
screenshot). As seen, I now show how the add to bundle UI looks when a
bundle exists for a user.

The screenshot below makes the change from radio button to dropdown to
account for the case where no change wants to make to the archived
state of a patch.
