New design for file system layout and code update

I know several people have asked about the file system design changes being pushed for Witherspoon and the related code update plans. Attached is some much belated documentation of the current design, and the new design proposal being implemented. It also includes some history of how the current system came to be, some documentation of the existing init ram Fs scripts, a glossary of terms, and describes the boot process at some levels. Not to forget, limitations and some motivations are provided.
