[PATCH] fix add notifier crashes

This is more or less the same as the patch I sent on 4 October.
It was called "crash-on-pci-slot-add.patch"

There's another closely related null ptr deref that is fixed in the
patch "rpaphp-crashing.patch" that was the next one in that series.

Anyway, other than that, it looks good to me.
