Series of patches to spread lpevents by default and related cleanups

Hi,

The following patches remove the lpq pointer from the paca,
then enable spreading of lpevents by default.

Then there's a number of cleanups on the code in
arch/ppc64/kernel/ItLpQueue.c and surrounds. Most of this should
only effect iSeries.
