[PATCH 4/11] PPC64: remove /proc/ppc64/{naca,paca/xx}

Hi Andrew,

This patch removes the (unused) /proc entries for the naca and the (per
cpu) pacas.  Also it removes a lot of no longer necessary includes of
<asm/naca.h>.
