Hit it again. It seems I have a bug in my application where im trying
to access just past the end of my allocated largepage memory region (16GB).

Somehow I end up getting a small page pte in there:

pmd_hugepage, pmd 4800b6c0 addr 10400000000
kernel BUG in hugepte_offset at arch/ppc64/mm/hugetlbpage.c:204!
