[PATCH 0/3] Traps/Head naming cleanup and log machine checks

These patches cleanup traps.c, head.S and adds support for logging
machine checks.

They mostly clean up the naming convention to more closely resemble the
Linux style.  I have built for pSeries, iSeries, and PMAC.  I have not
boot tested an iSeries and Olof is going to boot test a G5 for me.  

trap-cleanup: Cleans up naming and moves machine dependent calls	     
to a ppc_md.  It also moves the pSeries code to a pSeries file.

head-cleanup: Cleans up naming conventions.

machine-check-logging: Originally from Dave Altobelli, it adds support  
for machine-check error logs.
