[PATCH] Specify right base address/IRQ for ttyS0{2,3}

Attached patch sets valid default values for the 3rd and 4th serial port on pSeries systems.

On the systems I have checked, they're always at 0x890/0xf and 0x898/0xe.

I suppose that in a perfect world we'd get the values out of the device tree, but with the current
serial code it'll be more work that it's potentially worth.

Patch is against ~2.4.21, I'll check it into 2.4/2.5 BK tomorrow unless I get suggestions for changes.
