How do external irq's get mapped?

Dear Andy:

I want to thank you and your group for getting us off the externalIRQ
block. Using irq = 128 allows the external device to interrupt on IRQ0
to the 8541.

I know it was a long, drawn-out explanation, but the help in
understanding is much appreciated.
