[PATCH v2 15/25] powerpc/io: Spell-out PCI_IO_ADDR

PCI_IO_ADDR is a ppc-ism, which obscures the fact that some of the
powerpc accessors are identical to the generic ones. So remove it and
spell out the type fully.
