[PATCH 0/4] powerpc/xive: fixes and debug extensions

Hello,

First two patches are fixes for non-critical issues. I checked that
they applied on stable.

Last two are debug extensions, one for xmon and the other to dump XIVE
internal state under debugfs, which is easier than xmon.
