[PATCH 13/16] powerpc/64s: Drop _MASKABLE_RELON_EXCEPTION_PSERIES()

_MASKABLE_RELON_EXCEPTION_PSERIES() does nothing useful, update all
callers to use __MASKABLE_RELON_EXCEPTION_PSERIES() directly.
