[PATCH 07/14] powerpc/pseries: improved exception vector macros

Add macros that can specify exception vectors more completely (location,
type, etc). Move most of the exception code naming out of exception-64s.h,
and feed labels to it directly (not quite complete yet).

Code is unchanged except for names. Alignment directives scattered around
are annoying, but done this way so that disassembly can verify identical
instruction generation before and after patch. These get cleaned up in
future patch.
