[PATCH v3] powerpc: Merge 32-bit and 64-bit setup_arch()

There is little enough differences now.

mpe: Add a/p/k/setup.h to contain the prototypes and empty versions of
functions we need, rather than using weak functions. Add a few other
empty versions to avoid as many #ifdefs as possible in the code.

 create mode 100644 arch/powerpc/kernel/setup.h

v3: Move empty definitions to arch/powerpc/kernel/setup.h, they're not needed
    by other parts of the kernel so it's neater to keep the private. Fix build
    break for SMP=n BOOK3E=y.
v2: Add empty versions using #ifdef in setup.h rather than weak functions.
