[RFC PATCH 2/4] Migrate cpu hotplug code to pseries/hotplug-cpu.c

This patch moves the cpu hotplug handling code from pseries/dlpar.c
to pseries/hotplug-cpu.c. Additionally it factors out the work to
add/remove a single cpu into its own routine.
