[ccan] New module: iobuffer

Here's a patch, to be applied after my initial one, that renames the
refill and flush callbacks, as well as *_ensure, all to just "need".
It also clarifies how callers and callees of the need callback (for
both ibuffer and obuffer) should behave with respect to unnecessary
invocation of the need callback.
