This problem seems to be found on the CECHH, CECHJ, CECHK, CECHL,
CECHM, and CECHP models when the PS3 internal HDD is formatted
for 100% GameOS.  This problem also causes kexec, shutdown, and
reboot to fail.  

I have a temporary workaround here:

ps3-petitboot-09.09.01 or later will have the workaround:

We are still working on the final solution.
